Search Form
  1. Search BarSearch Bar
    1. Enter a location in the search bar. Enter as much information as possible about the location you want to use as the center of your search (street, city, state, ZIP code, etc.).
      1. For addresses, enter the physical street address, not a Post Office Box number or Rural Route number.
      2. Do not enter apartment or unit numbers as a part of the address; only the street address is needed.
      3. As you type in the search form, the tool will give you a list of location suggestions based on what you have typed. When you see a correct match, you can tap on it to complete the entry.
    2. Once your search query has been submitted, a green arrow will appear on the map at the location you entered. Depending on filter criteria, nearby facilities will be visible on the map as dots.

    Tapping on this button automatically sets the Search Bar to your estimated GPS location. You must allow the application to use your devices current location for this feature to work.

  1. Facility Types
  2. Depending on your needs, the Locator allows you to search for substance use treatment facilities, mental health treatment facilities, or both substance use and mental health treatment facilities. To do this, tap either SU (Substance Use Facilities), MH (Mental Health Facilities), or SU & MH (Both Substance Use and Mental Health Facilities). Note: SU & MH is the combination of both substance use and mental health treatment facilities; this includes all treatment facilities known to the Locator. It is NOT limited to facilities that offer both mental health and substance use treatment at their location. The default search for the Locator is SU & MH.
  3. Service Code Shortcuts
  4. The purpose of this section is to be a shortcut for selecting commonly used service codes without having to find and toggle the service code within the Service Codes Section (3). Currently, the only service code in this section is U.S. Department of Veterans Affairs (shortcut button appears as Veterans Affairs). Selecting this service code will filter results to show only facilities operated by the U.S. Department of Veterans Affairs.
  5. Additional Facility Types
    1. Health Care Centers
    2. Tapping on this icon will additionally show Health Resources and Services Administration (HRSA) health centers. These centers can provide care, even if you have no health insurance. You pay what you can afford, based on your income. Health Care Centers provide:
      • Checkups when you are well
      • Treatment when you are sick
      • Complete care when you are pregnant
      • Immunizations and checkups for your children
      • Dental care and prescription drugs for your family
      • Mental health and substance use care if you need it
    3. Buprenorphine Physicians
    4. Tapping on this icon will additionally show physicians (and their locations) who are authorized to treat opioid dependency with buprenorphine.
  6. Service Codes, Geo Restriction, and Filter/Sort
  7. Service Codes, Geo Restriction, and Filter/Sort button These 4 buttons provide options to refine your search. Tapping on them opens a dropdown menu containing options. If any one of the 4 has an active refinement/restriction applied the color of the button will change to navy blue as shown in the image above. For more information about the filtering options contained in each button, please view the Service Codes Section (3), Languages Service Codes Section (4), Geo Restriction Section (5), and Filter/Sort Section (6) help topics on this page (below).

Service Codes Section
You may want to only search for facilities that offer specific services (e.g. outpatient care or payment assistance). To only show facility results corresponding to specific service codes, tap on the desired service codes within the Listing section. The size of the listing section may be reduced by either toggling Show Only Selected, entering search text in the Service Code Filter Bar, or by tapping Show More/Less Services.

    Toggling this on will reduce the size of the Listing Section by only showing the services that are currently selected.

    You may reduce the service codes visible in the Listing Section by entering search terms into this filter element. The listing will only show Service Codes containing the text entered into this element.

    By default, only the most commonly used/offered service codes are shown in the Listing Section. If you would like to view the full list of service codes, you may tap on the Search More Services button. To go back to the default list, tap Search Less Services (same button).

    This section lists possible services that facilities offer. You may select service codes by tapping on them. When service codes are selected, only facilities offering that service are shown. Selected service codes will be highlighted and have a checkmark on the right hand side. Tapping on selected service codes will deselect them. Additionally, service code categories are collapsible/expandable by tapping on the service code category itself.

Languages Section
You may want to only search for facilities that offer specific language services (e.g. services for the Deaf and Hard of Hearing, American Indian or Alaskan Native Languages, and Other Languages). To only show facility results corresponding to specific languages, tap on the desired language service codes within the Listing section. The size of the listing section may be reduced by either toggling Show Only Selected, entering search text in the Languages Search Bar, or by selecting languages from the Listing section.

    Toggling this on will reduce the size of the Listing Section by only showing the language services that are currently selected.

    You may enter search terms into this filter element. The listing will only show Languages containing the text entered into this element. To the right of the search bar you may click on the 3-dots icon ‘additional options’ to select from a dropdown menu to Expand All, Collapse All, and Deselect All.

    This section lists possible language services that facilities offer. You may select specific languages by tapping on them or select all by clicking on the All button. When languages are selected, only facilities offering that service are shown. Selected languages will be highlighted and have a checkmark on the righthand side. Tapping on selected languages will deselect them. Additionally, language categories are collapsible/expandable by tapping on the language category itself.

Geo Restriction Section
Geo Restriction Section
You may limit your search results to only show results within a specified amount of miles, within a state, or within a county. Only one of these restrictions may be selected at a time. Tapping on the selected restriction will deselect it.
Distance in miles
Only show facilities within a certain amount of miles from the entered location (Green Arrow on Map). You can either choose a preset amount of miles (5, 10, 15, or 20), or enter the amount of miles by tapping Custom and typing into the box that appears. A red circle will appear on the Map outlining the search area.
State
Only show facilities within the state currently entered in the Search Bar. A red line will appear outlining the state’s border in the Map.
County Only show facilities within the county currently entered in the Search Bar. A red line will appear around the county’s border in the Map.

  1. Sort By
  2. This will order the matching facilities in the Facilities View (10) by a certain criteria (e.g. show closest facilities first, put them in alphabetical order, etc.). Tapping on the Clear button will reset Sort By to the default (Distance: Low to High).
  3. Filter By
  4. You can refine your search by entering text into the facility name, facility address, and phone number boxes. The text input in these search boxes may be a partial word (e.g. north) contained within the facility name, address, or phone number. After tapping Apply, only facilities containing the text entered in their respective Names, Addresses, and/or Phone Numbers will be shown. Tapping on Clear removes the facility name, facility address, and phone number text and performs a search again.

Tap the L icon in the top right of the map page to open a Legend showing which colors correspond to certain facility types. Once opened, you may tap the “hide” button to hide the legend.

Tapping on this icon will simply center the map on the location currently entered in the Search Form (1). This icon is NOT for changing the Search Form to your current location (for that function, use the Current Location Button in the Search Form (1)).

This button is for conveniently expanding/collapsing the Facility Filters Section (2). This is the same as tapping on the heading bar for the Facility Filters Section.

Map View
Map View Icon After submitting a search query, colored dots representing matching facilities appear. To see which dot colors correspond to certain facility types, please consult the Map Legend (6). You can tap on the individual dots to get more information about the facility. With the facility information visible, you can tap “View in List” to switch to the Facilities View (10) with the corresponding facility highlighted.

    The facilities that appear on the map based on your search will be listed here along with their address, phone number, and distance from your search location. Tap the “View on Map” button to switch to the map view (with the map centered on the selected facility).
    • To go to a facility's homepage, tap the "Website" link of that facility (some homepage’s may not be available)
    • Tap on "Directions" to view a description of how to locate a facility from the location currently entered in the Search Bar. Additionally, you may open directions in Google Maps by tapping the Google Map Icon.
    • Tap "More Info" for any facility to view more details about the facility and the specific services offered.

They will allow you to enter a search location; choose between substance use treatment facilities, mental health treatment facilities, or both substance use and mental health facilities; search for Health Care Centers; search for facilities operated by the U.S. Department of Veterans Affairs; search for facilities with specific services; limit your search by state, county, or distance; and/or use the Options link to use more advanced functions.   • 2. Facility List Tool Bar

    Example of the Facility List Tool Bar in the Locator Map page Using this tool bar, you can download your search results in a Microsoft Excel® spreadsheet, share your search results via email, print your search results, and/or filter/sort your results. Go To Top

You can click on a facility in the list to highlight the facility on the map. Beneath each facility, links for Directions and More Information appear. Directions will give you driving directions from your search location to the address of the facility. More Information will show the specific services offered by the facility. Go To Top

Initial Search from the Home Page

    3. As you type in the search box, the tool will give you a list of location suggestions based on what you have typed. When you see a correct match, you can click on it to complete the entry.
  2. When you are ready, click the Go button.
  3. The site will take you to the Locator Map page, where a list of facilities and a map with dots showing the location of facilities will appear.

Hints and Tips

  • If no results are returned, zoom out on the map to show a larger area. Or, check the Distance box and enter a larger number.
  • If you search only with a ZIP code, county, or state, the tool will search outward from the geographic center of the area. Thus, you may get results for other areas as well. You can limit the search area by searching for a more specific location or using the check boxes in the “Find Facility” search form to limit your search by distance, county, or state.
  • You can change the number of results displayed on a page by selecting a new number in the “Show [number] per page” box at the bottom of the facility list.
  • If two or more dots overlap on the map, you can zoom in to look at a smaller area and separate the dots.
